Dear Parent/Guardian,

We understand that the After School Program is a valuable resource to many families, and we are looking forward to continuing our After School Program for the 2022-23 school year.

For the health and safety of our scholars and staff, we will continue to align with the practices of Cresthaven Academy Charter School to the greatest extent possible, and we will abide by all state and federal regulations.

If you would like to enroll your scholar in the 2022-23 CAF After School Program, please ensure that you have a $0 account balance in Procare. If you need help checking your Procare balance, contact afterschool@cresthavenacademy.org.

You must complete and submit all enrollment documents for your scholar, along with a non-refundable $25 per child enrollment fee.

If you have an outstanding Procare account balance, registration fees will go toward your outstanding balance, and additional payments will be required to enroll your child.

Space in the After School Program is limited, and registration is accepted on a first come, first served basis. If enrollment is full at the time of your application, your scholar will be placed on a waitlist in the order their application was received. Waitlisted families will be informed of their scholar’s position on the waitlist, and contacted if/when space in the program becomes available.

We recommend that you complete the registration process online, however if you need to pick up or return a paper copy of the enrollment forms, please contact us.
